2. not be related to the person subject of the affidavit (e.g. not a parent, child)
3. have personal knowledge of the facts subject of the affidavit.

This affidavit is for discrepancies in your name. If the discrepancy is only for your birth date or both name and birth date, then use the following affidavits instead:

2. When do you need a Joint Affidavit of Two Disinterested Persons?

This affidavit is a requirement when you apply for late registration of birth, marriage, or death of a person.
